What are the requirements needed for the employee to apply?

1. Application Form:
   1. To apply for a loan, employees need to complete the below application forms:
      • Application Form: Wellness Financing
      • Application Form: Salary Loan
2. Application Requirements:
   1. Borrower needs to submit the following:
      • Fully Accomplished Application Form
      • Identification Document (TIN)
      • Valid ID
      • Bank account must be the name of the applicant - emphasize that we can also disburse to e-wallets (e.g. GCash) if applicant has no bank account
      • If unable to provide, we can disburse to company’s (SME) bank account
